Any time you would like us to know something about your culture, lifestyle, or identity, please let us know. We will protect the private information within your Health Record and you will always be informed of the purpose for collecting any information.
Health Records will only be shared if you request it or if required by law (such as in an emergency). If necessary, you can ask us to make corrections to your Health Record. The Star Health Client Health Records Policy can also be obtained by contacting our Quality Officer at 132 246 (132 BHN).
