Dementia Support Service

People living with dementia, their families, and/or carers are supported by the Dementia Support Service.

A multidisciplinary team of medical specialists, occupational therapists, and other allied health professionals provide specialised and practical support.

The Dementia Support Service includes:

  • An assessment of physical, social, and emotional well-being
  • The development of a care plan based on strengths, interests and independence
  • Providing assistance to the client, family or carer in understanding their condition
  • Help people with dementia live well by providing them with practical strategies
  • Provide assistance and navigation to a range of community support services
  • Management and minimisation of behavioural and psychological symptoms of dementia. These could include wandering, aggression and anxiety
  • Continence management, safety around the home, and general health advice
  • Group and individual education sessions
  • Referral to other community services
  • Prepare a person for the future in regard to driving, employment, legal issues, care arrangements and medical care
  • Monitoring on an ongoing basis as needed
  • Providing referrals to other services

Who can use the service?

  • People with dementia or facing cognitive decline
  • Eligible participants in the Commonwealth Home Support Programme (CHSP).
